About Pathways to Housing Program





Radius’ Pathways to Housing Program is designed with the goal of ending homelessness for people who suffer from complex psychiatric illnesses, usually co-occurring with substance use issues, along with significant medical and social challenges. The program aims to:

- Provide immediate access to permanent independent housing without requiring treatment or sobriety as a precondition for housing.
- Offer community-based outreach support and treatment, founded on harm reduction principles and client choice.
- Deliver services based on evidence-informed best practices and contributing to such evidence to find new and effective approaches to recovery, integration, and community involvement.

Job Summary

The Pathways to Housing Program Support Assistants provide a variety of functions including coordinating Team and Client activities and schedules; reception, clerical tasks and other assistance to meet Client and Team needs; preparing program reports; providing a central communication hub for staff communication and safety; supporting electronic client records and troubleshooting.



These tasks all require that Program Support Assistants maintain a base line knowledge of each client’s current situation, recovery goals and case management plan.

Key Responsibilities

Team Support:

Attend daily team meetings to stay up-to-date on client status and provide support as needed (e.g., scheduling appointments, relaying information).

Reception & Client Interaction:

Greet clients and handle phone interactions, addressing their concerns or directing them to the appropriate team member.

“Command Central”:

Serve as the central communication hub for the team, coordinating activities, troubleshooting problems, and initiating emergency protocols when necessary.

Client Services:

Assist with client-related tasks, such as confirming appointments, arranging meetings, running errands (e.g., picking up goods/materials), or helping clients shop and attend appointments.





Financial Support:

Assist with client financial matters, liaising with financial managers and third-party income sources, and supporting the Office Manager with record-keeping.

Administrative Tasks:

Provide clerical support, including scanning, filing, updating records, making calls, and preparing meeting minutes, agendas, and reports.

Database Management:

Maintain electronic client files and databases, troubleshooting data entry issues and generating reports as needed.

Program Reporting:

Prepare and maintain various program reports (e.g., Excel, PowerPoint), including collecting and analyzing data for special projects.

IT Support:

Troubleshoot issues with technology and office equipment (e.g., computers, cell phones).

Confidentiality:





Ensure compliance with confidentiality requirements under the Health Information Act and maintain secure paper and electronic files.

Other Duties:

Perform additional tasks as assigned by the Program Director.

Qualifications

Education:

A degree or diploma in Human Services, Health, or a related field. An equivalent combination of education and relevant experience may also be considered.

Technical Skills:

Advanced computer skills (Windows Office Suite, database software, electronic client files, and mobile technology).

Financial Knowledge:

Basic understanding of bookkeeping principles.

Relevant Experience:

Experience working with individuals with mental illness, addictions, trauma histories, and/or homelessness. Knowledge of Housing First and Harm Reduction principles is an asset. Experience working with diverse cultural populations, including Indigenous peoples, is also an asset.

Teamwork & Communication:





Proven ability to work effectively in a team environment, with strong organizational and communication skills (both written and oral).

Personal Attributes:

Punctual, responsible, detail-oriented, and a continuous learner.

Driver's License:

Valid driver's license (non-GDL).
