The salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.

Providence Healthcare is seeking Licensed Practical Nurses to join our Crosstown Clinic team on a casual basis. About Crosstown Clinic:
Crosstown Clinic is a pioneering leader in substance use treatment, recognized as the first clinic in North America to offer diacetylmorphine (prescription heroin) to individuals with severe substance dependence for whom conventional treatments have proven ineffective.



The clinic focuses on delivering Injectable Opioid Agonist Therapy (iOAT) and provides life-changing care to clients who require this specialized treatment approach. You can learn more about Crosstown here.

The Role:
Reporting to the Patient Care Manager or Resident Care Manager and collaborating with an interdisciplinary team, the Licensed Practical Nurse (LPN) plays a vital role in delivering nursing care in accordance with Providence Health Care’s (PHC) mission, vision, and values. LPNs function independently while recognizing when patient acuity or circumstances require collaboration with RNs or RPNs for clinical intervention.

Key Responsibilities:

- Assist patients/residents in meeting their physical, social, spiritual, and psychological needs.
- Implement established care protocols and standards of practice for LPNs in British Columbia.
- Actively participate in care planning, assessments, interventions, and evaluations.
- Document care activities and advocate for patients/residents’ needs.




- Attend care conferences and interdisciplinary team meetings.
- Support the orientation and training of new staff and students.
- Respond to and report incidents per established procedures.
- Perform clerical duties as required.

Education:

- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
- Current basic cardiac life support certification.

Skills & Abilities:





- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
- Physical ability to carry out the duties of the position
- Ability to apply decision making and analytical skills
- Ability to role model conflict resolution strategies
- Ability to prioritize and organize work
- Ability to teach and identify patient/resident learning needs

Your Day to Day

- Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities.
- Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed.
- Prepare patients for procedures, transfers,



and admissions, coordinating with the interdisciplinary team.
- Administer medications per protocol, monitor effects, and maintain communication with the team.
- Utilize evidence-based resources to stay updated on nursing practices.
- Respond to incidents such as codes, injuries, spills, and alarms, following procedures.
- Assist with staff orientation, demonstrating procedures and providing support.
- Address patient/resident and family learning needs, provide educational materials, and make referrals.

- Attend care planning meetings, provide input, and advocate for patients/residents.
- Collaborate with the team to develop and evaluate individualized care plans.
- Assist with discharge planning based on patient status and external supports.
