The Opportunity:

Under the direction of the Therapeutic Lead, Clinic Solutions,



the role of the Practitioner is responsible for the delivery of patient services offered by Sentrex Health Solutions. The Practitioners main responsibilities include, but not limited to managing day to day patient care for assigned clinics. This role involves direct interactions with patients, and collaboration with members of an interdisciplinary health team such as family physicians, therapists, pharmacists, insurance companies, and other health care professionals (HCP’s) to plan, implement coordinate and evaluate patient care in consultation with patients and their families, as well as support the day-to-day operations and expansion of the program through the development of professional relationships with key stakeholders and community providers within selected geographies.

Clinical Responsibilities:





- Has professional and managerial responsibility for a caseload of patients in which they are responsible for decision making and clinical judgement to formulate appropriate treatment plans in consultation with affiliated physicians
- Perform a patient advocacy role
- Prescriber and administer medications
- May co-ordinate or manage primary patient care
- Responsible for developing and implementing nurse-led clinics within the overall service provision for patients. The practitioner will work in partnership with physicians, nurses, consultants and the multidisciplinary team to provide best possible care for patients.
- Act as a primary point of contact to their assigned health care providers (HCP) and ensure patients are provided easy access to their prescribed therapy. In addition, this role involves direct interactions with patients, insurance companies, pharmacies,



physicians and other health care professionals (HCPs) to explore and obtain coverage for the prescribed medication
- Examine patients and take their histories, order laboratory tests, x-rays and other diagnostic procedures, and consult with other medical practitioners to evaluate patient’s physical and mental health
- Prescribe and administer medications and treatments
- Perform and assist in routine procedures, including lumbar punctures
- Advise patients and their families on health care including health promotion, disease, illness and accident prevention, counseling patients and their families on a range of health and lifestyle issues

Operational Responsibilities:

- In partnership with the senior leadership team, develop, monitor, and lead the implementation of the clinical services strategic and operational plans,



and provide input and monitor applicable financial budgets.
- Oversee the creation and implementation of program objectives, action plans, along with benchmarks and evaluation procedure that align with the strategic and operational plans of the business.
- Demonstrate implementation of the organizational core values and find new ways to embed the organizations core values within the clinical services team.
- Develop, motivate, and coach new employees under the clinical nursing services department

A Day in the Life (What you will do here):

- Diagnosing, Assessing, Managing patients.
- Process data entry activities of pertinent information received at different points of contact during the delivery of services
- Provide medical support to patients and HCPs, including but not limited to education on specific diseases, therapies, and disease management.
- Prepare for and attend extensive,



specialized training sessions for specific therapies.
- In person and virtual support (including adherence support both inbound and outbound as needed)
- Review patient files prior to treatment to capture medical history
- Document and maintain patient records to ensure treatment history is adequately captured
- Maintains confidentiality of patient and corporate information and discusses same only with appropriate personnel
- Liaise with physicians’ offices, pharmacies, services providers, and other parties to provide comprehensive and coordinated service to patients and HCPs.
- Participates in ongoing internal and/or external continuing education activities
- Hosts patient education based on specific patient needs, individually or in group settings.
- Responsible for scheduling appointments




- Participates in telephone consultations and virtual meetings/training sessions as required.
- Always adhere to professional nursing standards.
- Work collaboratively within multidisciplinary health care team providing oversight
- Other duties as assigned. Performs other related duties and tasks as required
- Read and review all Canadian product monographs to ensure a comprehensive understanding of each product being administered
- Report and document Adverse Events as per training as per Pharmacovigilance requirements
- Provide ongoing support on disease state or product related questions.
- Adhere to Sentrex Policies and Procedures
- Proactively offer support and advocacy to patients and HCP which includes but is not limited to enrollment, facilitation of coverage renewals, and changes throughout the patient journey,



as well as facilitating reimbursement support to physicians and patients.
- Follow and abide by all provincial College of Nurses guidelines while providing patient care.

What you need to ensure you are set up for success:

- Minimum 2-4 years of clinical experience (preferred)
- Licensed Nurse Practitioner or General Practitioner with experience in geriatrics, neurology or GAIN/GEM
- Must have an active license to work in Ontario
- CPR and First-Aid certification required
- Able to confidently deliver education including but not limited to patient self-injection education
- Ability to confidently administer medication IV, IM and or SC
- Experience in critical care, emergency and acute care  is a strong asset
- Confident and experienced with using Excel, Outlook and Word
- Able to work autonomously with little supervision, as part of a larger remote team
- Able to travel within a set geographical boundary
- Experience in Patient Support Programs is a strong asset
- Understanding of the pharmaceutical industry will be considered an asset
- Strong analytical and mathematical skills
- Effective interpersonal skills
- Strong organizational skills and attention to detail
- Ability to meet deadlines and be flexible with patient and HCP requirements
- Excellent problem-solving skills; ability to resolve issues effectively and efficiently
- Ability to communicate effectively both orally and in writing
- Adhere to Sentrex Policies and Procedures
- Bilingual English and French an asset but not required
