As a Social Worker, Youth Mental Health and Substance Use Case Coordinator, you will play a vital role in supporting young people facing mental health and addiction challenges, working under the guidance of the Patient Resident Care Manager and the Professional Practice Leader of Social Work.

Key Responsibilities:

- Interprofessional Collaboration:



Work as part of an interprofessional team to provide clinical services to individuals, primarily youth and young adults, affected by mental health and addiction issues.

- Client Coordination: Coordinate the intake and transition of clients through the continuum of care. Assess clients, determine referral needs, and provide individual counseling, intensive case management, group therapy, and education. We work with vulnerable youth who are homeless, unstably housed, experiencing mental health concerns and substance use. Our practice is guided by the main theoretical frameworks of Harm Reduction, Attachment Theory, Trauma Informed and Cultural Humility. This work can take a Case Manager to meet a youth where they feel most safe. This can include their single room occupancy buildings, shelters, hospitals, detox, treatment centers, shared community resources or on the street.

- Treatment Planning: Formulate and implement treatment plans. Provide comprehensive case consultation and follow-up support on treatment decisions in collaboration with interdisciplinary team members.

- Team Participation: Participate in team and local committees,



internal and external case conferences, and quality improvement activities.

- Community Collaboration: Collaborate with other community service providers to deliver education and training related to mental health and addiction.

- Program Development: Provide input into the development, evaluation, and delivery of services at Foundry Vancouver Granville.

This role is essential for supporting the well-being of our clients and enhancing the effectiveness of our services.

Education and Experience Requirements:

- Master's Degree in Social Work from an accredited university.
- Two (2) years of recent, related experience working with youth and young adults with mental health and substance use challenges in a community health setting, or an equivalent combination of education, training, and experience.




- Current full registration with the British Columbia College of Social Workers.

Skills and Abilities

- Knowledge of Addiction and Mental Health: Demonstrated understanding of addiction issues, mental health challenges, and evidence-based treatment practices.

- Assessment Proficiency: Ability to conduct comprehensive psychosocial assessments using standardized tools for mental health and substance use.

- Crisis Management: Skilled in managing complex clinical and psychosocial crises within the youth and young adult population.

- Interpersonal Communication: Strong ability to establish rapport with diverse clients and communicate effectively with individuals and teams in a multicultural environment.

- Organizational Skills: Capable of organizing, prioritizing workloads, and developing educational resources and support groups.

- Knowledge of Relevant Legislation:



Comprehensive understanding of relevant Acts, professional practice guidelines, and community resources for effective referrals and consultations.

- Hours:



Monday to Friday, from 9:00 AM to 5:51 PM. Case Managers work on an Earned Day Off (EDO) schedule, allowing them to take every 10th day off by working longer hours.

Your Day to Day

- Therapeutic Counseling: Providing clinical counseling services, helping clients explore personal concerns, and facilitating changes in their coping styles.

- Psychosocial Assessments: Conducting interviews and gathering data on a client’s environment, family, and finances to enhance coping strategies and care planning.

- Crisis Intervention and Support: Offering immediate support during crises, assessing emotional responses, setting goals, and teaching coping mechanisms.

- Care Plan Implementation: Creating and following through on care and treatment plans with a team, ensuring they respect the client's independence and dignity.

- Client Coordination:



Managing client intake, transitions, and referrals to ensure they receive the appropriate services in the continuum of care.
