The salary range for this position is CAD $41.42/Hr. - CAD $55.91/Hr.
Reporting to the Operations Leader or designate, the position works as part of an interprofessional mental and/or addiction health care team to provide flexible and inclusionary treatment and intensive case management services needs to individuals, primarily youth and young adults, with, and affected by mental health and addiction; and to provide integration and continuity across the care continuum for individuals with mental illness and/or addiction. Establishes and maintains relationships with, and consults with, clients, their families and colleagues to provide appropriate treatment services based on best practices and client need. Performs key duties such as intake,
mental health assessment, treatment planning, crisis intervention, consultation, education, and support to facilitate and coordinate client access to services. Participates in team and/or local committees, internal and external case conferences and quality improvement activities. Collaborates with other community service providers and related disciplines to provide education and training related to mental/addiction. Provides input into the development, evaluation and delivery of the Inner City Youth Program.
Education and Qualifications:
- Registered Nurse (RN) with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM), or
- Registered Psychiatric Nurse (RPN) with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Minimum of two years' recent, related experience working with youth and young adults affected by mental health and addiction in a community health setting.
Skills and Abilities:
- Mental Health and Addiction Knowledge:
Demonstrated knowledge of nursing theory and practice related to mental illness and addiction.
- CPR Proficiency: Demonstrated ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
- Resource Knowledge and Liaison Skills: Knowledge of hospital and community-based resources, with the ability to act as a liaison between services to facilitate client access.
- Psychosocial Theories: Knowledge of Early Psychosis Intervention and Attachment Theory, and their application in client care.
- Treatment and Crisis Management: Demonstrated ability to provide treatment planning, crisis intervention, and effective case coordination.
- Complex Crisis Management: Ability to manage complex clinical and psychosocial crises within the identified population, ensuring timely and effective support.
- Interdisciplinary Collaboration:
Knowledge of other healthcare disciplines and their role in supporting client care, along with the ability to work collaboratively as part of an interprofessional team.
- Rapport Building: Skilled at establishing rapport with multi-barrier youth and young adult populations to foster trust and engagement.
- Education and Training: Ability to teach clients, families, and staff, enhancing their understanding of mental health, addiction, and treatment options.
- Decision Making and Prioritization: Strong analytical skills and the ability to make informed decisions. Capable of prioritizing and organizing work effectively in a dynamic environment.
- Client Care Technology: Ability to operate related equipment and client care information systems accurately and efficiently.

Your day to day
- Provides outreach services to youth and young adults with chronic psychiatric issues and/or addiction, using an intensive case management approach. This includes receiving and responding to referrals, evaluating client needs, and developing or revising treatment plans.
- Assesses high-risk situations and provides crisis intervention when required, meeting with clients in various settings such as emergency rooms, inpatient units, or residences to deliver client counseling services. Collaborates with the interprofessional team to implement optimal care plans.
- Establishes and maintains therapeutic relationships with clients and their support network, promoting self-care, informed decision-making, and empowerment to improve clients’ quality of life. Engages stakeholders in treatment planning and evaluation and provides education on managing and treating psychiatric conditions.
- Delivers treatment services including crisis intervention, family work, and education. Utilizes a variety of theoretical models, such as assessing emotional responses to illness, teaching coping strategies,
and collaborating with colleagues for comprehensive care.
- Initiates and participates in case conferences with clients, families, service providers, and the multidisciplinary team to address mental health and addiction issues, coordinate services, and ensure follow-up care is well-coordinated.
- Acts as a client advocate to protect and promote autonomy, privacy, dignity, and access to information, ensuring clients' rights are respected throughout their treatment journey.
- Collaborates and consults with colleagues to provide clinical support for continuity of care, specialized services, and community resource coordination, helping clients navigate healthcare and social programs, as well as assisting with forms, transportation, and daily living tasks.
- Maintains accurate client records in accordance with standards, documenting progress notes,
significant events, and referrals to ensure compliance with regulatory requirements and support the treatment plan.
- Participates in committees, task groups, and community initiatives, providing clinical insights and advocating for improved mental health and addiction services. Promotes the integration and coordination of services for clients and their informal support network.
- Engages in nursing orientation, shares clinical knowledge with staff and nursing students, and contributes to interdisciplinary team development.
- Takes part in planning, implementing, and evaluating client care policies and procedures, supporting nursing practice councils, clinical pathway development, and other program activities.
- Supports nursing research by identifying potential clinical research projects, adapting research to practice,
and participating in activities aimed at enhancing nursing practice.
- Performs other related duties as required.
